Why VO2 max predicts longevity

A 2018 study in JAMA Network Open following 122,000 patients found that low cardiorespiratory fitness (CRF) was the strongest predictor of mortality — comparable in magnitude to smoking, and stronger than hypertension, diabetes, or high cholesterol.[1] Moving from the lowest to the second-lowest fitness quartile reduced mortality risk by 50%+.

VO2 max reflects the integrated capacity of your heart, lungs, blood, and muscles to deliver and use oxygen. It declines ~10% per decade after 30 without deliberate training. Preserving it is one of the highest-leverage longevity interventions available.

How to measure VO2 max

Lab VO2 max test (gold standard)
Maximal exercise test with metabolic cart. Most accurate. Available at university exercise physiology labs, sports medicine clinics, and some gyms. Typically $150–300.
Wearable estimate (Garmin, Apple Watch, WHOOP)
Algorithmically estimated from heart rate and pace during outdoor runs. Reasonably accurate for tracking trends (±10–15% vs lab). Sufficient for most optimization purposes. WHOOP →

Run as far as possible in 12 minutes on a flat surface. Use a calculator to convert distance to estimated VO2 max. Not as accurate as lab but useful for tracking change over time.

How to improve VO2 max

Zone 2 training — 2–4 hours/week
Low-intensity aerobic work at conversational pace. Builds mitochondrial density and cardiac output. The foundation of VO2 max development. Requires patience — improvements show over weeks to months.
High-intensity interval training (HIIT) — 1–2 sessions/week
4×4 minute intervals at 85–95% max heart rate with equal rest. The most time-efficient VO2 max stimulus. Best layered on top of zone 2 base, not as a replacement.
