What DEXA measures

Dual-energy X-ray absorptiometry (DEXA) uses two low-dose X-ray beams to distinguish between bone mineral, lean tissue, and fat tissue throughout the body. A full scan takes approximately 10 minutes and produces:

  • Total lean mass (muscle + organs) by region
  • Total fat mass by region (including visceral fat estimate)
  • Bone mineral density (BMD) at spine, hip, and whole body
  • Body fat percentage with high precision (±1–2%)
  • Visceral adipose tissue (VAT) score — the metabolically active fat around organs

The scale shows weight. DEXA shows whether that weight is muscle, fat, or bone — and where the fat is distributed.

Why DEXA matters more than BMI or scale weight

BMI is a ratio of weight to height — it cannot distinguish between muscle and fat. A well-muscled 40-year-old and a sedentary 40-year-old of identical height and weight have dramatically different health risk profiles. DEXA reveals the difference.

More importantly, visceral fat — the fat stored around internal organs — is the metabolically harmful fraction. Subcutaneous fat (under the skin) carries much lower risk. Only DEXA and CT imaging can quantify visceral fat; the scale cannot.


Who should get a DEXA scan

Adults over 40 pursuing body composition goals
Baseline DEXA before a fat loss or muscle building protocol gives you actual data to track progress against — not just scale weight.
Women approaching or in perimenopause
Bone loss accelerates significantly as estrogen declines. Baseline BMD after 40 is a clinical standard of care that many women never receive.
Anyone with a family history of osteoporosis
Genetic predisposition to bone loss warrants earlier and more frequent BMD monitoring.
People with normal BMI who feel metabolically off
"Normal weight obesity" — normal BMI with high body fat and low muscle — is surprisingly common and only DEXA reveals it.

How to find a DEXA facility

Search "DEXA scan near me" — facilities include university exercise physiology departments, sports medicine clinics, radiology centers, and some dedicated body composition studios. Cost is typically $50–150 and is rarely covered by insurance for body composition purposes (bone density scans may be covered for qualifying women).

Use the same facility and machine for repeat scans to ensure consistent comparison.
