Apolipoprotein B (ApoB) is a protein that sits on the surface of every atherogenic (artery-clogging) lipoprotein particle in your blood — including LDL, VLDL, and IDL. One ApoB protein per particle means ApoB is a direct count of total atherogenic particles.
Standard lipid panels measure LDL-C (the cholesterol inside LDL particles). Two people can have identical LDL-C but very different ApoB — because LDL particle size and number vary widely. The person with more, smaller LDL particles has higher ApoB and higher risk, even at the same LDL-C.[1]
Every major cardiovascular endpoint study that has compared LDL-C to ApoB has found ApoB to be a superior predictor of atherosclerotic cardiovascular disease (ASCVD) risk.[2] The European Atherosclerosis Society and other leading bodies now recommend ApoB as the primary lipid risk marker.
