High ApoB: What It Means and What to Change First

What elevated ApoB indicates

ApoB above 80 mg/dL means you have more atherogenic lipoprotein particles in circulation than is optimal. These particles — primarily LDL, VLDL, and IDL — are the ones that infiltrate arterial walls and initiate atherosclerosis. The higher the ApoB, the greater the exposure your arteries experience over time.[1]

Elevation can result from dietary factors, metabolic dysfunction, genetic predisposition (familial hypercholesterolemia), or a combination. The intervention depends on which driver dominates.

What not to overreact to

  • A single test in isolation. ApoB can vary by 5–10% between readings — repeat after 4–6 weeks before drawing conclusions.
  • ApoB in the 80–95 mg/dL range in someone with no other metabolic risk factors, active lifestyle, and low inflammation. Context matters.
  • ApoB elevation alone without considering the full metabolic picture (hsCRP, fasting insulin, blood pressure, HbA1c).

Immediate actions

1

Check your full metabolic panel

Pair ApoB with fasting insulin, hsCRP, HbA1c, and triglycerides. High ApoB with elevated fasting insulin suggests metabolic syndrome — a different driver than genetics. Quest Health →

2

Start omega-3 supplementation

2–4g EPA+DHA/day. Best-evidenced supplement for lowering ApoB and triglycerides. Allow 90 days to see full effect. Momentous Omega-3 →

3

Reduce refined carbohydrate intake

Dietary refined carbs and sugars drive VLDL production — the upstream precursor to ApoB-containing LDL. This has a faster effect than reducing dietary saturated fat for most people.

4

Increase zone 2 aerobic activity

150+ minutes/week of low-intensity cardio improves hepatic lipid clearance and reduces VLDL output.

5

Retest at 90 days

If lifestyle changes don't move ApoB below 80 mg/dL after 90 days — discuss with your physician. Statin or PCSK9 inhibitor therapy may be warranted depending on overall cardiovascular risk.

When to see a doctor

ApoB above 100 mg/dL warrants a clinical conversation regardless of other risk factors. ApoB above 130 mg/dL may indicate familial hypercholesterolemia — a genetic condition that requires medical management and cannot be resolved through lifestyle alone.

High ApoB: pair particle count with inherited risk context

ApoB tells you how many atherogenic particles are circulating now. Genetics can add context around cholesterol handling, inflammation, and cardiovascular predisposition. SelfDecode is useful when you want a more personalized risk picture before choosing interventions.
